Description
Workers/Drivers must check the ‘Fault & Repair Report Book’ daily as part of the ‘Safety Inspection’. All previous recorded faults and repairs, either completed or being monitored are to be recorded in this ‘Fault & Repair Report Book’. This process is a form of communication that is required internally between Management, The Scheduler, Workers/Drivers and the competent/responsible person to document and communicate the progress of any recorded faults or faults that are being monitored.
During the ‘Safety Inspection’ or any journey, Workers/Drivers are required to record any Faults or Repairs as and when they are observed. A created record of any fault is to be documented in the ‘Fault & Repair Report Book’. Workers/Drivers are responsible to report all recorded Faults or Repairs to The Scheduler, or as per any instruction or training provided. The Fault & Repair Report page number of the completed record is to be noted in the ‘Pre-Trip Record Book’ in the right side column. This is for administration and tracking purposes. Instruction is provided within the ‘Pre-Trip Record Book’ on how to complete any record. You must inform The Scheduler of any recorded faults that may affect the safe operation of the vehicle or configuration. This is to comply with the reporting obligations.
When completing a record, each section of the page is to be fully completed with as much detail as possible to assist with the assessment and repair of recorded Faults or Repairs. When a fault is recorded, the white page is to be removed from the ‘Fault & Repair Report Book’ and forwarded to The Scheduler, workshop or competent/ responsible person for processing and traceability.
IMPORTANT – during any inspection, if the Worker/Driver observes that a vehicle has an open fault, contact is to be made with The Scheduler prior to commencing the journey. Faults or Repairs as observed during the ‘Safety Inspection’ or when being repaired or serviced, may affect the safety of the vehicle or configuration. Unsafe vehicles or configurations must not be moved or driven at any time.
