FORM 44 Subcontractor Prequalification Questionnaire

The FORM 44 – Subcontractor Prequalification Questionnaire is to be completed by a subcontractor and submitted back to the operator along with all supporting documents and records as requested within the form. This form shall then be reviewed and completed by the person considering the subcontractor for work i.e. Director/s or nominated person.
All mandatory requirements that are highlighted on the form must be satisfactorily addressed and evidence provided before a subcontractor can be recommended for work.

Where an assessment shows some deficiencies, a Subcontractor may still be considered by adopting the operators system to cover the gap.
