FORM 5 Annual Internal Review Mass
The Annual Internal Review form is to assist transport operators in meeting requirements under NHVAS Mass Management, relevant to the undertaking of Annual Internal Reviews. When completing this Annual Internal Review, you will be required to assess all the information as recorded in the completed Monthly Compliance Reports from the date of the last completed Annual Internal Review.
As and when conducting the Annual Internal Review, all records and processes are to be assessed based on the potential risk that may be
imposed if the operator is not conforming to internal processes, procedures and NHVAS Business Rules & Standards.
The risk matrix as recorded at the rear of this document is to be used to assess any Non- Conformance as raised during the Annual Internal Review and the required actions or improvements required are to be recorded and consulted with The Operator/Business.
