FORM 3 Training Awareness and Education Record

The Training, Awareness & Education Record records the training completed for all your staff that are under your internal management systems for NHVAS, WAHVA and/or Chain of Responsibility.
This form is to be completed with knowledge and understanding of the Induction
Training Competency Matrix (FORM 3 – Part 2). All sections within the Induction Training Competency Matrix have advised responsibilities and processes that need to be referenced to within the training provided, as and when required.
