This form is to be completed in advance of rostering & scheduling any workers/drivers to the known available work.

The record is to be completed progressively if the periods of work across a day or days, and all information is to be entered as and when known, and verified on completion against available records.

Transport operators can refer to the Safe Operating Procedure (SOP 58) and/or information as obtained when completing the TLIF0005 & TLIF0006 Training Courses for NHVAS and/or the WA WorkSafe Administrator Online Level & Driver Level Training when completing this record.
