FORM 26B Verification of Competency – Plant and Equipment

This assessment form is to be used to assess and report on the competency of a worker/driver or operator being assessed on the Operation of Plant & Equipment.
The assessor must record an assessment mark against all lines when the person being assessed has been observed performing the task or process.
This form is appropriate for all transports operators regardless of their accreditation status.
