FORM 6 Letter of Authorisation

A Letter of Authorisation is to be carried in the vehicle by the nominated and approved worker/driver at all times when operating under the NHVAS Fatigue Management Accreditation.

This form and the accreditation certificate may be requested to be produced by the Worker/Driver as proof to the regulatory authorities on request. Workers/Drivers that have been authorised under The Operator’s/Business’s accreditation and who have been issued this letter, must also be recorded in the Training Needs Analysis & Register FORM 2 and Training, Awareness and Education Record FORM 3.
