FORM 15 Record of Competency and Table of Tolerances

This resource is to be utilised to obtain record and confirmation to ensure a ‘Competent & Responsible Person’ maintaining your vehicles is assessed and deemed competent to conduct the required or scheduled maintenance.
An assessment by a Suitably Qualified Person is to be performed and recorded in this form, as evidence that the persons named below have been consulted and assessed through this process and are deemed competent at the time of the assessment.

On the rear of this form is a Table of Tolerances & Authority to Maintain Accredited Vehicles which documents maintenance tolerances aligned with the National Heavy Vehicle Inspection Manual V3.0.

The Form 15 also includes a section on ‘Authority to Maintain Accredited Vehicles’ where maintenance providers nominated for the undertaking of scheduled maintenance, servicing and repairs on the accredited vehicles and configurations are recorded and approved.
