FORM 18 Statement of Compliance (Mass)

This form is to be issued to suppliers of services to ensure they are aware for the requirements of suspension maintenance of the units supplied.

The vehicles listed in the schedule (overleaf) are being maintained by The Operator/Business in accordance with Standard 8 of the Mass Management module of the National Heavy Vehicle Accreditation Scheme (NHVAS). Namely that their suspension systems have been maintained and replaced according to appropriate specifications and codes as identified in the schedule.

NOTE: If the accredited Sub-Contracted vehicle is also accredited under the Maintenance Management Module, a Statement of Compliance is not required.
