FORM 12 Work Diary Register

This form is used to record the inspection and review of each National Driver Work Diary pages received from workers/drivers.
It is crucial for a transport operator to ensure individual serial numbers of the Work Diary Pages are recorded from the number of the first page and through to the last page with all records held.
Periodically as part of the system review, a new record must be inserted and these work diary numbers are to be checked to ensure consecutive numbering is maintained, and records are complete and accurate with no fatigue breaches.
This will assist transport operators in identifying ,issuing work diary pages, allowing for these to be requested from the worker/driver.

Any fatigue or work diary breaches identified through the use of them Work Diary Review Register can be recorded as a Non- Conformance using FORM 8.
