FORM 17 Periodic Weight Verification Register

This form is to be used to record the verification of vehicles or configurations gross and individual axle weights when loaded.
This process of verification needs to be completed at least bi-annually to ensure that the allowable mass limits for a vehicle accredited under the NHVAS/WAHVA Mass Management Module can be accurately adhered to.

The weights are to be determined on a calibrated weighbridge and this will be used to verify and calibrate the fitted weighing devices. When weighing devices are calibrated on each recorded date, the gauge readings are to be marked and recorded below.
