FORM 43 – Incident Record Verification Summary

This form is to be completed as soon as practical after an incident has occurred, in succession of the Form 41: Accident, Incident & Near Miss Report Form & Form 42: Incident Investigation Report.
The requested detail in each section of this form is required to be completed if applicable, and where known.
This form assists in reviewing & verifying all available records & documentation relevant to the vehicle/s & workers/drivers involved in an incident.
