FORM 4 Vehicle Resources Register

The use this form is to assist transport operators to record details of ALL issues vehicle resources, including the TransAssist Pre-Trip Record Books, TransAssist Fault & Repair Report Books, and NHVAS Interception Report Books (if applicable). These books when returned, must be filed for a minimum of three (3) years with all records, and this resource can assist in monitoring this.
This form will keep track of the date of issue, as well as the page numbers within the resource, who issued the resource, and the date of return.

This resource also has an incorporated QR Code to allow access to further instructions and help in completing this resource.
