FORM 35 Mass Record Sheet for Rigids

This form is to be used by Workers/Drivers when operating vehicles under the NHVAS Mass Management Module to document all loaded trips for RIGIDS completed when above General Mass Limits (GML) on an Axle Group or Gross Weight, or when travelling on a permitted route.

This form instructs Workers/Drivers to always check air gauge weights with brakes released and full air pressure. This allows for Workers/Drivers to tick the ‘Verified’ column as noted when weights are checked on a certified weighbridge against the in-vehicle weight gauges, and this is to be completed at any time when the opportunity permits.

If the Worker/Driver identifies during the loading process and assessment of Gross & Axle Masses that they are in breach of the ‘Allowable Gross & Axle Mass Limits’ documented at the bottom of the page, they are instructed to ensure the load is to be adjusted to comply prior to leaving the site.
